Branching Out Supported Services is a Medicaid Waiver Service provider for individuals with disabilities needing assistance with day to day living activities in the home, community, and place of employment. Supported Living Services, Day Habilitation Services, Supported Employment Services, Respite Services, and 24 hour care are available.
